This study is a Research Volunteer Registry. Volunteer Registries are secure databases of people who are willing to let researchers contact them when a research study needs someone like them. This is a Registry for people willing to be contacted about research being done by the Duke Vaccine and Trials Unit (DVTU).

If you join the Registry, you will:

• Sign this consent
• Share some limited information about yourself (name, phone numbers, email address, street address, and preferred method of contact, date of birth, race, gender, and ethnicity, height, weight, smoking status, childbearing potential (women only), and diagnosed health conditions.
• Agree to be contacted about future studies the DVTU is conducting (you do not have to join any of these studies)

The only risk of joining this registry is the possible loss of your privacy. We make every effort to protect your privacy but it cannot be guaranteed.

You are being asked to join this research project because you have agreed to participate in another study the DVTU is doing, or you learned about the project from other sources (word of mouth, advertisements and so on).

Research projects include only people who choose to take part. Please read this consent form carefully and take your time making your decision. As your study doctor or study staff talks with you about this consent form, please ask him/her to explain any words or information that you do not clearly understand.

We encourage you to talk with your family and friends before you decide to take part in this research project. The nature of the project, risks, inconveniences, discomforts, and other important information about the project are listed below. If you are participating in any other DVTU studies, your participation will not be affected by your decision whether or not to join this research project.

If you decide to join the Registry, Dr. Emmanuel Walter will be your doctor for this research project.

We are building this Registry to keep a database of volunteers who might be interested in future research studies conducted by the Duke Vaccine and Trials Unit (DVTU).

The DVTU staff will use the information collected about you and entered into the registry to contact you about participating in a future research study that may be of interest to you.

We may also use this information to contact you in the future about your research experience or to update your contact information.

If you agree to join this Registry, we will ask you to sign this consent form.
With your consent, we will collect and store your contact information for future research studies you may be interested in. 

The information we will collect includes:

• Contact information (limited to: name/preferred name, phone numbers, email address, street address, and preferred method of contact).
• Demographic information (date of birth, race, gender, and ethnicity).
• Profile information (limited to height and weight, smoking status, and (women only) childbearing potential).
• Any diagnosed health or medical condition(s)
• If you have ever received medical care at Duke or been in a study at Duke, we will ask you for your medical record number (if known).

There is a potential risk to your privacy. We make every effort will to maintain your privacy, however this cannot be guaranteed.

There will not be any direct benefits to you if you decide to join this Registry. There are no provisions for you to receive financial compensation if you join the Registry.

We will keep the information you provide for the registry in a secure electronic database on the Duke Network. After we collect the data, only designated DVTU staff will be able to access your information using password protected computers on the secured Duke Network. Your name, and any other information you provide, will not be shared with unauthorized persons.

Study records that identify you will be kept confidential as required by law. Federal Privacy Regulations provide safeguards for privacy, security, and authorized access. Except when required by law, you will not be identified by name, social security number, address, telephone number, or any other direct personal identifier in study records disclosed outside of Duke University Health System (DUHS).

The information retained in the database may be reviewed in order to meet federal or state regulations. Reviewers may include representatives of the Duke University Health System Institutional Review Board. If any of these groups review information in the database, they may also need to review your entire medical record.

If this information is disclosed to outside reviewers for audit purposes, it may be further disclosed by them and may not be covered by the federal privacy regulations. Summary information from this database may be presented at scientific meetings or published in a scientific journal, however, your identity will not be revealed.

If you agree to participate, the personal information you provide today that is added to our registry will be kept forever. If you change your mind and decide that you do not want to participate in the registry, we will change your status in our database to ensure you will no longer be contacted. No new information about you will be collected for the registry other than data needed to keep track of your withdrawal, and you will not be contacted for future studies.

There are no costs to you for joining the Registry.

You will not be paid for joining this Registry.

Immediate necessary medical care is available at Duke University Medical Center (DUMC) in the event that you are injured as a result of your participation in Registry. However, there is no commitment by Duke University, Duke University Health System, Inc., or your Duke physicians to provide monetary compensation or free medical care to you in the event of a project-related injury.

If you agree to join the Registry, you are free to change your mind at any time. If you decide to withdraw your permission to use your data in this research project, please contact Dr. Walter in writing and let him know you are withdrawing your permission for your information to be stored and used for future research.

"The purpose of this study, procedures to be followed, risks and benefits have been explained to me. I have been allowed to ask questions, and my questions have been answered to my satisfaction. El propósito de este proyecto es mantener una base de datos del registro de voluntarios que podrían estar interesados en estudios futuros de investigación realizados por la Duke Vaccine and Trials Unit (DVTU). El personal del DVTU usará la información recolectada sobre usted y la ingresará al registro para comunicarse con usted sobre participar en un futuro estudio de investigación que podría ser de su interés.

No existirá ningún beneficio directo para usted si decide participar en este proyecto de investigación. No hay disposiciones para que usted reciba una compensación financiera si decide participar en este proyecto de investigación.

La información que usted provee para el registro será guardada en una base de datos electrónica segura en la red informática de Duke University. Después de la recolección de los datos, solamente el personal designado del DVTU podrá acceder a su información. Este acceso se limita a personas específicamente autorizadas usando computadoras protegidas con contraseñas en la red protegida de Duke. Si usted acuerda participar, la información personal que usted provee hoy que se agrega a nuestro registro será mantenida indefinidamente. Si usted cambia de idea y decide que no desea participar en el registro, cambiaremos su estatus en la base de datos para asegurarnos que no lo contactaremos más. No se recogerá nueva información sobre usted para el registro excepto los datos necesarios para tener en cuenta su retiro y no se lo contactará para estudios futuros.

No hay costos para usted por participar en este proyecto de investigación.

No existe ninguna disposición que le provea compensación financiera si decide participar en este proyecto de investigación.

Si usted acuerda participar en este proyecto, está en libertad de cambiar de idea hasta que termine este proyecto de investigación.
