Thank you for your interest in joining the Duke-UNC Alzheimer's Disease Research Center ADD-ME Roster. The Roster is a great way to hear about studies that may be of interest to you.

The next step is to fill out the following:

Registration Form: gathers basic information, including your name and contact information.
Consent Form: provides detailed information about the Roster. If you agree to join the Roster, you will be asked to give your consent by providing an electronic signature. You will then be asked to certify your signature.
Demographics Form: collects information about your race, ethnicity, education, and memory and health conditions.
