PI Eligibility Request Form

The below form must be completed by an individual with appropriate rights and privileges within SPS (typically a departmental business office official). To be complete, the form must contain:

Justification of the request, demonstrating the qualifications of the proposed individual as PI to oversee and conduct the proposed activities
CV of the proposed PI candidate
Letter of approval from owning org unit leadership, signifying their endorsement of the request

Upon submission, school and management center officials will review the form and contact the submitter for any further details needed.

If the request is approved, the proposed individual will receive a notification email, with a copy to the submitter. Management center officials will be responsible for ensuring that institutional systems reflect the proposed individuals eligibility to serve as PI on research project applications for external funding.
